Having Trouble Signing a Form? |

How do I print my form? Blank forms cannot be printed. To print your form, you must first fill out some information on the form, click the submit button on the bottom of the form and follow through the rest of the process. At the end of the process you will be able to view, print and/or email the completed form. This limitation is required to protect our copyright rights; the unauthorized reproduction of forms is prohibited. Once a form has been filled out, it may be reprinted or photocopied for the intended use. See Also |

How do I save my form? Click the submit button on the bottom of the form. Submit does not transmit the data to the licensing authority; it simply saves the data you entered on the form. Note: You do not have to fill out the entire form at this time. You can partially fill out the form now, click submit which will save the data on the form as a draft, you can then come back and finish the rest of the form later. [More Info]. To save the form as a file on your hard drive, click the submit button on the bottom of the form and follow through the rest of the process. At the end of the process you will be able to open the completed form by clicking the view link on the My Forms page. Once the form is open, click File | Save As… to save the form to your hard drive. Note: You will not be able to modify the form after it is saved to your hard drive, this limitation is required to protect our copyright rights. See Also |

How do I sign my form? You cannot sign the form at this step in the process. If you chose the Submit Online option, then the site will take you through the process of signing the form on a subsequent page. If you chose Print and Mail, then you should sign the form by hand after it has been printed. Note: Signature and signature related fields are marked with a red asterisk (*), those fields are currently inactive. See Also |

I filled out my form, what do I do next? Click the submit button on the bottom of the form. Follow the instructions on the subsequent pages to complete the process. If you are not ready to continue complete the process now, click submit which will save the data on the form as a draft, you can then come back and finsh the rest of the process later. If I close my browser, how do I get back to my form? You can always return to the My Forms page by clicking on "Open Your Forms Now" on the invoice that was emailed to you or by visiting our web site, clicking on My Forms (located on the top right of the menu bar) and entering your order number and email address, you will find your order number on your invoice. |

I'm trying to print the form I purchased and nothing happens when I click on the print button The view and print buttons only become enabled after you have filled out the form. Click "Fill-Out" to open the from, enter your information, click the submit button on the bottom of the form and follow through the rest of the process. Once you have completed and reviewed the form, the form then becomes final and you will be able to view and/or print the completed form. Please note, the site does not allow the printing of blank forms. The unauthorized reproduction of our forms is prohibited. Once the form has been filled out, it may be reprinted for the intended use. |

I'm clicking on Fill-Out and the form does not open, I get a blank white screen or the browser freezes. Here are some common problems you may run into when opening PDF files
There are several possible causes to these problems - The most common reason is that the Adobe Reader software may be attempting to download an update. Look for an update dialog that may be hidden behind the browser window, follow the instructions and then try again. If you are still having trouble opening your form please email customer service, or call us at 845.369.8799 (Option 4). See Also |

This usually indicates that you are using an old version of Acrobat or Adobe Reader, we recommend that you use version 7.0 or later for the best user experience. Our forms require you to have Adobe Reader 5.0 and above or Acrobat 5.0 and above installed on your computer in order to be used as intended. I'm trying to sign a form using the mouse or the 'Generate Signature from Name" feature and nothing is happening. This usually indicates that the Java Runtime Software is not properly installed on your computer. In most cases you will notice the message "error on page" in the status bar (located in the lower left of your browser window). Please Verify your Java Software Installation and follow the instructions provided (link opens in a new window). Note: Java Runtime usually comes preinstalled on your computer and is not adware/spyware etc. If you have to install Java and need to close your browser, you can always return to you purchased forms by clicking on "Open Your Forms Now" on the invoice that was emailed to you or by visiting our web site, clicking on "My Forms" and entering your order number and email address.
