  • When attempting to open a PDF file, the browser becomes a blank white screen
  • The browser freezes and the PDF does not open

There are several possible causes to these problems - The most common reason is that the Adobe Reader software may be attempting to download an update. Look for an update dialog that may be hidden behind the browser window, follow the instructions and then try again.

How It Works: Step-by-Step

The process is a bit different depending if you purchased your form(s) with the intent to Print and Mail or if you selected the additional service to Submit Online.

« Print and Mail »

1. Fill out your form.

Note: You do not have to fill out the entire form at this time. If you simply want to print the form and fill it out by hand at your convenience, enter some information on the form (e.g. company name, owners name etc.), leave the rest of the form empty and continue with the following steps, you will then be able to print your form and fill out the rest of it on your own.

2. You cannot sign the form at this time. You will sign the form after it has been printed.

Note, signature and signature related fields are marked with a red asterisk (*), you should complete those fields after you print your form.

3. Click the submit button on the bottom of the form.

Clicking the submit button does not submit the data to the licensing authority; it simply saves the data you entered on the form as a draft, you can then come back and finsh the rest of the form later. [More Info]

4. You will now see the Filing Info page. On this page you can name your form, and enter or confirm the licensing authority filing fee.

This is optional, if you simply want to print the form you can just click continue, the information on this page will be used to generate a cover sheet with filing instructions which will become available when you print your form.

5. You will now see the Confirm page. On this page you can review the data you entered on the form and on the previous pages, modify or accept it as is. Click Complete to continue.

6. You will now be taken back to the My Forms page where you will be able to print your form by clicking on the Print link.

« Submit Online »

1. Fill out your form.

Note: You do not have to fill out the entire form at this time. You can partially fill out the form now, save it as a draft and finish later. [More Info]

2. You cannot sign the form at this time. The site will take you through the process of signing the form on a subsequent page.

Note, signature and signature related fields are marked with a red asterisk (*), those fields are currently inactive and will be filled in on a subsequent page.

3. Click the submit button on the bottom of the form.

Clicking the submit button does not submit the data to the licensing authority; it simply saves the data you entered on the form.

4. You will now see the Filing Info page. On this page you can name your form, and enter or confirm the licensing authority filing fee.

5. You will now see the Confirm page. On this page you can review the data you entered on the form and on the previous pages, you can still modify the form at this point. Click Complete to continue.

6. Once you clicked Complete, you can no longer modify data on your form, you will now be asked to sign the form. Once you sign your form, your signature will be placed on the form and the signature associated fields (e.g. date, title etc.) will be filled in automatically.

7. You will now be taken back to the My Forms page where you will be able to view, print or email your form. You can also fill out other forms you purchased (if any).

8. Once you have filled out all your forms, click the Submit link. This will send your forms to us for processing.

9. After clicking submit you will be prompted to pay the licensing authorities fees.

10. Once the fees are paid, we will process your application. One of our specialists will review the application and look for missing information or incorrect fees, we will contact you if there is an issue with your application, we will then send the application and the licensing fees to the proper authority an you will be notified that the application has been submitted.
